Please complete the form below and submit by email to Traci Malloy.

  • Facility Use Forms are required for all reservations.
  • Forms should be submitted a week in advance to ensure that HVAC will be ordered in a timely manner.
  • All changes / cancellations must be submitted a week prior to the scheduled event.

Please Note: All non-school groups must have a Certificate of Liability Insurance in the name of your organization for a minimum amount of 1,000,000 dollars in order to rent any GCPS facility.
