Calling all Comet Athletes! Need your sports physical for next year?

Northside Hospital's Orthopedic Sports Medicine dept. will be offering these for $25 by appointment only at the following two locations:

Sat., 4/18 @ Brookwood & Meadowcreek High Schools * Register by Mon., 4/13

Sat., 5/2 @ Mill Creek & Central Gwinnett High Schools * Register by Mon., 4/27

All proceeds go to the school you selection in registration. 

Register @ Source-Gwinnett. NO WALK UPs or REFUNDS.

Each athlete is required to fully complete the Preparticipation Physical Evaluation (PPE) sections prior to arrival:

• HISTORY FORM (page 1 & 2)
• Signature of athlete and parent/ legal guardian on (page 2)
• Shared Emergency Information section (page 4)
• GFL/REC: Bring your own paperwork
